What are ‘Action Plans’? Action cards tell a member what are the specific actions need to be taken by the member to address the gaps in his/her Medical care so as to optimize his/her care. Member can print an additional copy of the report and take to the doctor’s office that his doctor can ensure that gaps in his health care can be addressed so that he is getting the most appropriate care as recommended by the experts in the field.
